I see that there is an smtp config and I successfully send a test email, but what can I configure to use this? It would be nice if watchdog could send an email that a particular port is down...
What else can be configured to use email alerts?

Well first there have been some fixes to SMTP alerts so I would run v1.4.0rc19

Currently all alerts will go out SMTP including ping watchdog

Or if running v1.4.0rc19 if a port speed changes you get an alert

Anything bad is sent out SMTP alerts

We probably should create a config page where you can select what is and is not sent, maybe v1.4.1
